Review Standard

Proof, Conflict, Consequence

Each item should ask what document proves the claim, what document contradicts it, and what rights were affected.

What did the record prove, what did it omit, and what decision depended on it?

Unsupported Allegations
Claims repeated without identified source evidence, witness foundation, medical record, financial record, or transaction record.
Verification Failures
Places where a material claim appears to have been accepted before independent records were reviewed.
Material Omissions
Known records, conditions, communications, or alternatives that affected the meaning of the submitted narrative.
Service and Notice Defects
Potential defects involving notice, service, hearing opportunity, recipient ability to review documents, or procedural participation.
Evaluation Problems
Questions about source materials, vision limitations, testing context, collateral information, assumptions, and evaluator access to treating records.
Procedural Momentum
Events where earlier unverified assertions appear to have carried forward into later actions.
